1944年10月生まれ。アイルランド南部クロナキルティ出身。英ブルース・シーンで長らく活躍を続けたブルース・ハープ奏者。Savoy Brownのオリジナル・メンバー。
1955年に両親とともにイギリスに移り、やがてジャズやブルースに関心を持つようになる。1962年にジャズ・クラブでAlexis Korner’s Blues Incorporatedのギグを体験、特にCyril Daviesのブルース・ハープに大きな影響を受ける。
1965年にはロンドンでKim Simmondsと出会い、同年10月にバターシーでSavoy Brown Blues Bandを結成。翌年シングルでメジャー・デビューを果たすもアルバム録音を前にグループを離脱。翌68年にはJohn Dummer Blues Bandに加わり、名作と評価の高いデビュー・アルバム「Cabal」の録音に参加した。
1969年には一大ブルース・セッション・グループSweet PainにDick Heckstall-Smithらと参加し、アルバム１枚を残している。前後してセッションでChampion Jack DupreeやThe Brunning Hall Sunflower Blues Bandのアルバムなどにも参加。
1970年代半ばは音楽からやや距離を置いていたが、1978年にSweet Painで一緒になった友人のKeith TillmanやDick Heckstall-Smith、Victor BroxらとThe Famous Bluesblastersを結成してシーンに復帰。グループはメンバーチェンジを経てMainsqueezeと名乗り本国をはじめ欧州をツアー、ライブ・アルバムも残している。
1980年代に入って同グループを脱退した後は、ロンドンを中心に様々なブルース・バンドで演奏。2001年にはギターのNeil Brown、オルガンのRob Millis、ドラマーのEd SpevockらとのThe Andy Cortes Bandを結成。2002年には自身のThe John O’Leary Bandを立ち上げ活動を続けた。
