1938年7月18日生まれ。スコットランド・エジンバラ北東のピッテンウィーム出身。The Rolling Stonesのロード・マネージャー兼セッション・ピアニストとして著名で「6人目のStones」の異名を持つ。ブギ・ウギ・ピアノの名手としてセッション歴多数。
ロンドン南部のサットンで育ち、6歳からピアノを学ぶ。1961年、Brian Jonesと出会いともに演奏活動をスタートさせ、翌年にはMick Jagger、Keith Richardsらと合流してThe Rolling Stonesを結成する。
順調に人気を得て晴れてプロ・デビューが決まるが、新マネージャーに就いたAndrew Loog Oldhamによって「ルックスがバンドに合わない」との理由でメンバーを外される。以降はロード・マネージャー兼セッション・ピアニストとしてバンドを陰で支え続けた。
Stonesがリリースしたオリジナル・アルバムの大多数にセッション参加しており、ライブ・サポートも担当。正式メンバーを外れたもののメンバーからの信頼は厚く、トラブル解決などバンドのまとめ役として大きな存在感を誇った。1985年に他界するまでStonesと行動を共にし尽力したが、彼の死でメンバー間に大きな溝ができ、バンド活動の停滞を招いたとも言われている。
セッション活動としてはStonesが所有していた移動スタジオ「モービル・ユニット」を管理していた関係でLed Zeppelinの傑作アルバム「Led Zeppelin IV」収録の「Rock And Roll」や「Physical Graffiti」に入った「Boogie with Stu」などへの参加が有名。ブルース系アーティストとの共演も多く、アルバム・プロデュースと合わせて同じくブルース・ピアニストのBob HallやThe Blues Bandらと度々共演した。
ソロ活動としては、自身の音楽的志向でもある古き良きジャズ＆ジャンプ・ブルースのプロジェクト・グループRocket 88が良く知られる。Stonesの仲間Charlie WattsやJack Bruce、Alexis Kornerら大物が参加、1979年から度々ライブ活動を展開し、81年リリースのアルバムではプロデュースを担当した。
ただ、このほかにリーダー作は少なく、Bobbie Millerとのカップリングによる66年のシングルＢ面でIan Stewart & The Railroaders名義の「Stu-Ball」がある程度。ミュージシャン仲間の間でその腕前が高く評価されるものの、自身の趣向であったブルースやジャズ以外の音楽には関心が無かったと言われている。
1985年12月12日、47歳の若さで心臓麻痺により他界。
